Not sure why, but I think the code "{{SMI HSC}}" (without the space, of course) breaks it, on the row where you mean to put the story codes. Edit: I think that the fact that the template page began with that noinclude code scuffed everything up... Not sure why, but I think the code "Template:SMI HSC" (without the space, of course) breaks it, on the row where you mean to put the story codes. Replaxing it with actual text at least displayed everything correctly when I tried preview page.
