It's not just about breath play. It's about helplessness, the loss of control, the risk, danger, uncertainty. I'm not normally into breath play either but I totally get it and I think this was well done. If her legs were unbound and her arms were tied in front, it would be easy for the guy to stand on the stool, put his head between her arms and then lift her up to fuck her.

This doesn't break the two rules

*You can only fuck her while she hangs
  • Her arms must be bound

If he were to do it this way, she wouldn't get any pleasure from being hung, but also can't do anything to him because he hasn't broke the rules.
