Bellatrix was born a Black, and married into Lestrange. It should be Paternal Aunt... unless she's illegitimate born to a woman with the name Lestrange. Rudolphus or Rabastan's sister. Could be Rabastan's daughter (Bellatrix's brother in law). That would still make it Paternal Aunt.
